Wow. And I thought I was paranoid about my eyes! *grin* This post hit on so many emotional points I can relate to. My eyesight is comparatively much better than yours, but my myopia and astigmatism is still so bad that I'm utterly useless without my glasses. Given how much I enjoy reading, working on a computer, etc...blindness IS and probably always will be my greatest fear.
All I know is that I think what you have decided to do is incredibly brave. You are taking a chance at changing your life permanently and irreversibly. It takes a very strong person to take the kind of step you are about to take. I have to say, I admire you very much for this.
